Young carers & early help services: case management and data

The digital backbone for young carers and early help services: referrals, assessments, support plans, safeguarding records, partner data sharing and outcomes reporting, delivered with the specialist providers who work directly with families.

Young carers and early help services depend on information moving safely between schools, social services, health and the organisations that support families. Too often it moves by email and spreadsheet: referrals arrive in inboxes, assessments sit in documents, and commissioners wait weeks for outcome figures that are assembled by hand.

We provide the case management, data sharing and reporting that sit behind these services. Direct support for young carers and their families is delivered by specialist providers with the right workforce and safeguarding arrangements; we work alongside them, as technology partner or subcontractor, so practitioners spend their time with families rather than on paperwork.

Who this is for

  • Specialist charities and VCSE organisations delivering young carers and family support
  • Local authority early help and children’s services teams
  • Commissioners who need consistent outcomes and contract monitoring data
  • Consortia bidding for prevention and early intervention contracts
Scope

What the solution covers

Referrals and early identification

A single referral route for schools, health, social services and families, with acknowledgements, triage and a clear record of who referred whom and when.

Assessments and support plans

Structured assessments and review cycles, with support plans, goals and agreed actions recorded against each young carer and family.

Safeguarding records

Concerns, decisions and escalations logged in one place with restricted access, timestamps and a full audit trail.

Activity and attendance

One-to-one sessions, groups, youth clubs, respite and school engagement recorded quickly by practitioners, including on mobile devices.

Partner data sharing

Information shared with schools, social services and health under agreed data sharing agreements, with role-based access and consent recorded.

Outcomes and contract reporting

Wellbeing, education and family resilience outcomes measured consistently, with commissioner reports produced from live records rather than spreadsheets.

Communications

A shared inbox for referrals and family contact, with ownership, response times and history kept against each case.

Security and governance

UK hosting, role-based access, audit logging, and UK GDPR roles and a data protection impact assessment agreed before go-live.

Delivery

How we deliver it

This describes how we run work of this type. It is method, not a completed contract.

  1. 1

    Discovery with practitioners

Referral routes, assessment forms, safeguarding procedures and reporting requirements mapped with the practitioners and managers who use them.

  2. 2

    Information governance

Data sharing agreements, consent model, retention periods and access roles agreed with partners and the commissioner before configuration.

  3. 3

    Configuration

Referral, assessment, support plan and outcome forms configured to the service model, with reports built against the contract’s monitoring requirements.

  4. 4

    Migration and training

Open cases migrated with checks, and practitioners trained in short, role-based sessions so the system is used from the first week.

  5. 5

    Live support and improvement

A named contact, agreed response times, and quarterly reviews of reporting against the outcomes the service is commissioned to achieve.

Risks we plan for

Safeguarding information in the wrong hands
Restricted safeguarding records, role-based access, audit logging and access reviews with the service lead.
Practitioners avoiding the system
Forms designed with practitioners, mobile recording, and short training before go-live.
Inconsistent outcome data
Outcome measures agreed with the commissioner up front and built into assessments and reviews.
Unclear data sharing between partners
Data sharing agreements and consent recorded before go-live, with sharing limited to what each partner needs.

FAQ

Questions buyers ask

Do you deliver direct support to young carers?

No. Direct support is delivered by specialist providers with the right workforce, safeguarding arrangements and experience. We provide the case management, data sharing and reporting, working alongside them as technology partner or subcontractor.

Can schools and health partners make referrals directly?

Yes. Partners can refer through a single route, receive an acknowledgement, and see the information they are entitled to under the agreed data sharing arrangements.

How do you produce commissioner reports?

From the live case records. Outcome measures and contract monitoring requirements are agreed at the start and built into assessments and reviews, so reports are produced without manual collation.

Can the system work in Welsh?

Bilingual Welsh and English delivery is part of our method for public services in Wales, covering forms, letters and messages as well as screens.
